Palliative and Supportive Care for Heart Failure

In two consensus conferences on Palliative and Supportive Care in Advanced Heart Failure, we developed a model of care for patients with heart failure.

This model, created by Dale Renlund, Sarah Goodlin and other pc-heart members, can be conceived of as two concurrent approaches: Heart failure care and Supportive Care. The specific interventions for each care approach depend on the “Phase” of illness. Together, the approaches represent comprehensive care. This graphic diagrams these concepts:

Thus, Heart Failure care is evidence-based pharmacological, interventional and surgical treatment specifically related to cardiovascular physiology. Supportive care incorporates “patient-centered care”, emphasizing the patient’s participation in decision-making and care as well as concepts from palliative care and hospice care. By conceptualizing a range of multidisciplinary interventions as “Supportive care” we address patient and family needs throughout heart failure management.

    Sarah,
    I am giving a talk 9/29 to a group of APN nurses—the title is above. I plan to use the PC-Heart comprehensive HF illness trajectory and discuss the consensus guidelines for advanced HF in the talk. Do I need your permission to use the graphic?
    Carol Flavell NP (Brigham and Women’s Hospital /Boston)and I were discussing developing a detailed basic presentation (perhaps funded or endorsed by PC-Heart) that could be presented locally/ nationally to HF disease management programs and Hospices. Let me know what you think— I can sent you the slide set of the APN presentation also if you would like to see in what context the graphic is being used.

    I signed-up as result of your talk in Austin – I would like to be part of the palliative medicine group – also I can’t seem to find you protocols that you use in the home for care of HF patients
